South Nassau Communities Hospital has selected Allscripts dbMotion(TM) and FollowMyHealth(TM) to provide system integration, data exchange, patient engagement and next-generation population health management and analytics to coordinate and improve patient care. In addition, South Nassau has also announced that it will become a Reference Site as part of Allscripts Reference Site Program. An Allscripts Reference Site partner is a health organization, system or office that utilizes an Allscripts solution or set of solutions in a best practices environment.

An Allscripts Reference Site allows healthcare IT professionals and decision-makers to see how the Allscripts Open and integrated platform can connect and help optimize patient care. South Nassau will use dbMotion to harness aggregated and harmonized data to improve care, care transitions, and efficiencies across its organization. The hospital will also implement the Allscripts Population Health Analytics solution, which provides analytics for high-cost, high-priority chronic diseases such as diabetes, heart disease, coronary artery disease and asthma.

The data will enable physicians to identify target populations, measure performance and outcomes, analyze utilizations, manage risk and access these insights within their workflow. In addition to helping manage care transitions, dbMotion 5.0 (Modular), an EHR Module, is compliant with the Health Information Technology Standards, Implementation Specifications and Certification Criteria for Electronic Health Record Technology, 2014 Edition in accordance with certain transitions of care certification criteria adopted by the Secretary of Health and Human Services supporting the Stage 2 meaningful use measures required to qualify eligible providers for funding under the HITECH Act. dbMotion has been certified by an ONC-ACB in accordance with the applicable certification criteria adopted by the Secretary of the U.S. Department of Health and Human Services.
