Stockholder Meeting Notice

The 2024 Annual Meeting of Stockholders of Surgery Partners, Inc. will be held on Thursday, June 6, 2024 at 2:00 p.m., central daylight time, virtually via the Internet at https://meetnow.global/MFATQXS. To access the virtual meeting, you must have the information that is printed in the shaded bar located on the reverse side of this form.

Proposals to be voted on at the meeting are listed below along with the Board of Directors' recommendations.

The Board of Directors recommends a vote FORall of the nominees, FORProposal 2, FORProposal 3 and FORProposal 4.

1. Election of Class III Directors:
   01 - Clifford G. Adlerz
   02 - J. Eric Evans
   03 - Blair E. Hendrix
   04 - Andrew T. Kaplan
2. Approval, on an advisory basis, of the compensation paid by the Company to its named executive officers.
3. Approval of the Surgery Partners, Inc. Employee Stock Purchase Plan.
4. Ratification of the appointment of Deloitte & Touche LLP as the Company's independent registered public accounting firm for the fiscal year ending December 31, 2024.
