The Board of Directors of POLMED S.A. with its registered office in Starogard Gdański makes public that on 27 January 2015 the Issuer was delivered the signed contract amendments for the provision of health care services concluded with the National Health Fund - Pomeranian Voivodeship Branch in Gdańsk (hereinafter: the NFZ). The value of benefits in 2015 resulting from the contracts amended is estimated by the Board of Directors at PLN 3,246,224.35.
The following amendments to the contracts with the NFZ were concluded:
1/ Amendment No. 9/2014 to the contract No. 11/00865/AOS/13/14 for the provision of health care services - outpatient specialist medical care, the maximum amount of obligations of the NFZ to the Issuer for the execution of the contract during the period from 1 January 2015 to 31 December 2015 was established at PLN 1,180,737.39. In accordance with the provisions of the amendment, the contract was given the number: 11/00865/AOS/13/15.
2/ Amendment No. 5/2014 to the contract No. 11/00865/ASDK/13/14 for the provision of health care services - outpatient specialist care (outpatient costly diagnostic services), the maximum amount of obligations of the NFZ to the Issuer for the execution of the contract during the period from 1 January 2015 to 31 December

2015 was established at PLN 155,662. In accordance with the provisions of the amendment, the contract was given the number: 11/00865/ASDK/13/15.

3/ Amendment No. 7/2014 to the contract No. 11/00865/REH/13/14 for the provision of health care services - rehabilitation - sets out the maximum amount of obligations of the NFZ to the Issuer for the execution of the contract during the period from 1 January 2015 to 31 December 2015 at PLN 1,244,544.96. In accordance with the provisions of the amendment, the contract was given the number: 11/00865/REH/13/15.
4/ Amendment No. 3/2014 to the contract No. 11/00865/STM/13/14 for the provision of health care services - dentistry, the maximum amount of obligations of the NFZ to the Issuer for the execution of the contract during the period from 1
January 2015 to 31 December 2015 was established at PLN 665,280. In accordance with the provisions of the amendment, the contract was given the number: 11/00865/STM/13/15.
Due to the nature of contracts concluded with the branches of the National Health Fund, it is not possible to determine in advance the value of the contracts for the entire period of its validity - it is only possible to determine it for the period, for which the maximum amount of obligations of the NFZ to POLMED SA was set out.
All the abovementioned amendments shall enter into force on January 1, 2015. Other conditions of the contract amendments do not differ from the commonly used conditions for these types of contracts.
The criterion for acceptance that the abovementioned contracts are significant is the fact that the Issuer in the last 12 months signed more contracts with the NFZ, which together meet the criterion of a significant contract, i.e. exceed 10% of the equity capital of the Issuer.

The total value of contracts for the provision of health care services signed with the NFZ for 2015 is estimated by the Board of the Issuer at PLN 6,608,700. However, it should be noted that this is only an estimated amount relating only to the small part of the whole package of agreements because amendments to other agreements with the NFZ branches, setting out the maximum value of

benefits for 2015 or capitation rates for other types of agreements, have not been concluded yet. The Issuer will inform about next agreements forthwith.
