Cabiling will join a panel session, “Improving Care Transitions Between Settings to Reduce Avoidable Readmissions and Improve Outcomes,” that takes place February 6, 2019 at 9:20 a.m., and will explore best practices for eliminating the potential for adverse events when patients move from one care setting to another.
